Instructions for New Patients

If you have not already scheduled your first appointment with us, please call our office and we will be delighted to answer questions and get you started.

Before Your First Visit

Thank you for booking a new patient appointment with NaturoMedica and welcome to our clinic. To assist the doctor in preparing for your appointment, we ask that you provide your medical history information prior to your visit. The secure form takes less than 15 minutes to complete and will be forwarded directly to your assigned doctor. Please be aware that the form will time out if you spend more than 30 minutes on a single page. You can't save your responses online to return to editing later, but if you need to leave the form before completing it you are free to start over at a later time.

Our basic health questionnaire is hosted on our secure messaging platform. You are not required to create a secure messaging account in order to fill out the questionnaire, but many of our patients find that it is a convenient way to communicate directly with their doctor. Please read below under "At Any Time" for instructions on setting up an account, as you will be prompted to verify your identity.

At Your First Visit

Please bring the following to your first appointment:

  • Photo ID
  • Insurance Card
  • Any recent lab results or medical records you think may helpful (optional)

At Any Time

We offer a secure messaging service which allows patients to send messages directly to their doctor, request prescription refills, and view basic lab results after they've been reviewed by their doctor. In order to access the full features of secure messaging, it is necessary to verify your identity when creating an account. You will be provided with the necessary information to set up and activate your account at the time of your first visit. If you have not yet had your first visit, you may create an account now but will need to postpone identity verification when prompted for Patient Registration. After entering your name, email, username, and password, you will be taken to a screen with three options to verify your identity and you'll need to choose

"I do not wish to complete my registration at this time. Please take me back to the homepage."

Our secure messaging platform can be accessed by clicking PATIENT LOGIN in the upper right corner of our site.
